It is important that you only complete these forms if asked to do so by practice staff and is used alongside a telephone or face to face appointment as additional information is likely to be required or need further discussion

Your doctor may ask you to complete a bladder diary if you are having problems controlling your bladder. 

Equally it may be helpful to bring this completed if you plan to discuss bladder problems at an appointment.
